Dear Sir/Madam,

I am writing to you with regards to the above as I have only just recently found out that I need to have my Certificate of Experiences verified before I can continue to pursue my Post-Graduate Training at Senior House Officer Position.

Please find enclosed, two Certificates of Experiences, one for my Medical and one for my Surgical House jobs with the names and signatures of the Consultants I have worked with.

I also would like to inform you that I am in the midst of trying to get my LR2 form sorted out for extension of my Limited Registration exempt from PLAB for my next posting. This is a Senior House Officer rotation in Medicine at Wigan and Leigh Hospitals NHS Trust commencing August 2nd.

I have actually sent off my LR2 form to them about 3 weeks ago, but the person in charge of Medical Staffing has resigned, causing a bit of a delay in returning my LR2 form, and I have been trying to contact them desperately. I will ensure they speed things up because I do understand that you have a heavy workload this time of the year and I wish not to cause you any inconvenience.
